Supporting People Across Forth Valley

Assertive Outreach Plus works across the Forth Valley region to support individuals aged 16 and over—as well as their families and communities—who are facing challenges related to substance use, health, finances, and social wellbeing.

We understand that substance use rarely exists in isolation. It’s often connected to wider issues such as mental health, housing insecurity, unemployment, financial hardship, and family disruption. That’s why our approach is holistic, flexible, and rooted in partnership.

We meet people where they are—whether that’s in their own homes, at community drop-ins with partner organisations, or via our Mobile Outreach Vehicle.

We aim to create lasting, positive change by offering practical, compassionate support tailored to each person’s needs.

• Challenge deprivation and its impact on life opportunities
• Work collaboratively using a whole-person, systemic approach
• Identify and dismantle barriers to treatment and support
• Deliver integrated support that addresses interconnected issues

We build trusted relationships with individuals, working together to identify the support they want and need. Our flexible approach means we adapt to changing circumstances and agree together how often we meet and where.

• Blood Borne Virus (BBV) testing
• Naloxone training and supply
• Injecting Equipment Provision (IEP)
